You must apply in person using Form DS-11 if at least one of the following is true:

• You are applying for your first U.S. passport

• You are under age 16

• Your previous U.S. passport was issued when you were under age 16

• Your previous U.S. passport was lost, stolen, or damaged

• Your previous U.S. passport was issued more than 15 years age

You must bring the following to successfully execute your DS-11 application

• Certified birth certificate with raised seal and both parent's names listed

• Personal identification such as a valid driver's license

• Checks. You will need a check for each DS-11 form and one for the execution fee which is paid to the library. The execution fee is set by the department of state and is currently $35. The library will accept cash for the execution fees and photos only. We do not accept credit cards at this facility.
